You will be asked to fill out the following forms prior to your first visit with us. For you convenience, you may open and print these forms from this site and fill them out ahead of time. Be sure to bring the completed patient registration, fitting questionnaire (choose either upper or lower extremity), financial agreement, return policy and acknowledgement of privacy practices. The privacy notice is included here for your records, but you do not need to bring it with you. For your first appointment, you will also need to bring your insurance card(s), one form of identification, and any written prescription or referral your doctor may have given you.
